Access Community Health awarded ACC national contract
Access Community Health has recently been awarded the ACC national contract to provide Integrated Home and Community Support Services (IHCS) for people throughout New Zealand who have sustained an injury and require community-based rehabilitation, nursing and support services. We are able to support clients in both the short or long term/lifetime for those who have a serious injury. We are pleased to extend our relationship with ACC and while this is a new contract Access has been working with ACC as a national lead provider since 2012.
All clients will be supported by our highly skilled workforce of Registered Nurses, Occupational Therapists, Physiotherapists, trained Support Workers and Care Coordinators who work within a multidisciplinary team environment. Our goal is to provide high-quality support to optimise our clients recovery from injury and to maximise their return to independence.
On receipt of a referral from ACC one of the Access registered health professionals will complete a holistic, culturally responsive face to face assessment and support plan. We will also work with the local care coordinator to arrange services and assign the most appropriate support team to enable their rehabilitation.
Access Community Health has offices across the country in all regions to provide a local support network for our clients.
